Define the present fitness concern

Start with function and risk, not a diagnostic label. Identify the clinical tasks, decisions or behaviours said to be affected; the period involved; evidence of harm or potential harm; and the registrant's current duties. A health condition that is well managed and does not impair work is not, by itself, a disciplinary concern. Performance difficulty may also be remediable within a practice. RCVS involvement becomes more likely where fitness to practise may be impaired, reasonable corrective steps are not taken or the underlying conduct could reach serious professional misconduct.

Separate employer capability, occupational health and RCVS processes. An employer decides contractual duties and workplace adjustments, an occupational clinician advises on function, and PIC decides the regulatory route. Obtain reports with informed consent and a defined purpose. A useful medical report addresses current functional impact, treatment engagement, prognosis, relapse indicators and safe adjustments without unnecessarily disclosing a lifetime medical history. For performance, gather objective case review, audit, observation and competence evidence rather than a manager's unsupported label of poor performance.

Design workable support and undertakings

The RCVS Health and Performance Protocols allow a managed approach in suitable cases. Possible undertakings may address treatment, workplace supervision, limits on particular procedures, competence assessment, reporting or information sharing. Draft each promise so compliance can be observed: name the supervisor, permitted work, report frequency, review criteria and action following concern. Confirm the practice can deliver the arrangement and that it protects animal welfare in everyday conditions, including nights, lone work, emergencies and changes of employment.

Confidentiality should be planned rather than promised absolutely. Identify which medical details remain with the treating or occupational clinician and which functional conclusions must reach RCVS, an employer or supervisor. Obtain clear consent for necessary reports and know how urgent safety information will be escalated. If the registrant cannot agree to or comply with a proposed condition, raise that before signing and propose a safer alternative. Breach of an undertaking or failure to cooperate can lead to renewed investigation or disciplinary referral.

Evidence progress and plan the review point

Create a review pack that shows change over time: treatment or occupational reports, supervision notes, targeted CPD, case audits, error trends, reflective work and feedback. Relate each item to the original risk. A list of courses or an unqualified testimonial cannot establish safe performance on its own. Record setbacks honestly alongside the response taken. The purpose is to demonstrate whether risk remains managed and whether undertakings can narrow or end, while recognising that PIC retains responsibility for the regulatory assessment.

Know when the supportive route may no longer be sufficient. Serious harm, dishonesty, refusal to engage, repeated unsafe practice or breach of undertakings may lead PIC to consider Disciplinary Committee referral in the public interest. A health explanation can be relevant without excusing every act or preventing formal action. At each review, request a written decision recording continuation, variation, closure or referral. Keep emergency contacts and independent wellbeing support active throughout; regulatory cooperation should never replace necessary clinical care.
